What is gynaecomastia?

Gynaecomastia is a medical term that refers to the enlargement of male breast tissue. Commonly known as ‘man boobs’ or ‘moobs,’ this condition is often caused by a hormonal imbalance between testosterone and estrogen.

This prevalent condition affects teenage boys and older men, but it can develop at any age. Approximately 33% of men will experience gynaecomastia at some point in their lives. Despite its commonality, the condition often remains undiscussed due to embarrassment and distress.

The size and symptoms of gynaecomastia can vary, ranging from a small enlargement of the breast tissue to a fuller, more female-looking breast. It can affect one or both breasts and may cause tenderness.

In 90% of teenage cases, gynaecomastia resolves on its own. However, for many men, it can persist. Diet and exercise alone cannot correct the issue, making male breast reduction surgery an increasingly popular option to enhance self-esteem and confidence.

What are the causes of gynaecomastia?

Several factors can lead to male breast enlargement in London, including hormonal imbalances, obesity, puberty, aging, and the use of certain drugs and medications:

  • Hormone imbalance: Gynaecomastia can occur when the female hormone estrogen becomes imbalanced with the male hormone testosterone. Often, the exact reason for this imbalance is unknown.
  • Being overweight: Obesity and being overweight can increase estrogen levels, leading to breast tissue growth. Although diet and exercise can help, they do not always resolve gynaecomastia.
  • Puberty: Hormonal fluctuations during puberty can cause a temporary drop in testosterone levels, allowing estrogen to promote breast tissue growth. This enlargement typically resolves as hormone levels stabilise with age.
  • Ageing: As men age, their testosterone levels naturally decrease, and body fat often increases, which can raise estrogen levels and lead to the growth of breast tissue.
  • Drugs and medications: Recreational drugs such as marijuana, cocaine, anabolic steroids, and alcohol, as well as certain medications, can disrupt hormone levels and cause gynaecomastia. Continued use of these substances can lead to recurring gynaecomastia even after surgery.

What are the types of gynaecomastia?

Gynaecomastia can be divided into 3 grades according to its severity;

  • Grade 1: Enlargement of the gland only and this is fairly rare.
  • Grade 2: Enlargement of the gland and the fatty tissue that subsides in the breast tissue.
  • Grade 3: The skin envelope is also enlarged and will require treatment.

What are the types of gynaecomastia?

  • Medication review: If medication is identified as the cause of breast enlargement, your medical practitioner may consider discontinuing the medication.
  • Treatment of underlying disease: If an underlying medical condition is contributing to gynaecomastia, addressing the underlying disease may be necessary.
  • Medication prescription: In some cases, medications such as Tamoxifen may be prescribed to regulate hormone levels.
  • Surgery: Breast reduction surgery involves removing excess breast tissue and fat to create a tighter, firmer, and more contoured chest. This procedure is typically performed under general anaesthetic, although it can sometimes be performed under local anaesthetic in mild cases.

How is male breast reduction carried out?

This largely depends on the severity of your gynaecomastia.

  • Grade 1: Usually performed under local anaesthetic and requires a semicircular incision made around the bottom half of the areola, through which the excess breast tissue is removed. The scar, once mature, is very difficult to detect.
  • Grade 2:  Performed under a general anaesthetic and uses the same surgical method as grade 1 but also requires liposuction/liposculpture.
  • Grade 3:  Performed under a general anaesthetic and usually involves some form of breast reduction. This is also referred to as a modified wise pattern scar. Occasionally patients require a free nipple graft.

Recovery from male breast reduction surgery largely depends on the type of gynaecomastia carried out. As a guideline, recovery is as follows:

  • Grade 1: One week
  • Grade 2: Two weeks
  • Grade 3: Two weeks is the usual recovery time but occasionally the T-junction can take a little longer to heal.

After your breast reduction surgery you will:

  • Wear a compression garment for six weeks, and
  • Receive regular follow up appointments with me to monitor your progress. You will remain under close observation for the first twelve months following surgery, after which you are expected to be discharged.

Major complications are extremely rare with this form of surgery and I will discuss your particular risks during your consultation. The major concern is of risk of recurrence and hence it is important to remove any factors that may be contributing to the formation of breast tissue such as high alcohol intake. For grade 3 gynaecomastia correction, there is a very small chance of losing sensation to the nipple areola complex.
